Cerebral microangiopathy (CMA) is one of the significant causes depression in elderly. Close associations risk developing with white matter hyperintensity, presence lacunar infarcts, and other markers vascular disease are shown. The available data suggest that various mechanisms, particular, involvement small vessels brain, generalized microvascular endothelial dysfunction, metabolic factors, – factors for development depression. Pathogenetic mechanisms include cerebral hypoperfusion immune dysregulation. Depression also a common complication coronavirus infection, occurring both acute postCOVID periods. same as involved pathogenesis post-COVID depressive disorders. Given complexity disorders patients CMA, severe comorbid pathology, antidepressants an optimal ratio efficacy safety should be preferred. Agomelatine (Valdoxan) such drugs.
